AUTHORIZATION FOR REPAIRS
I hereby authorize Lions Auto Repair INC ("Lions Auto Repair") to perform all required repair work on my vehicle along with the necessary materials. I grant Lions Auto Repair INC and its employees permission to operate the vehicle on streets, highways, or elsewhere for the purpose of testing and inspection. An express mechanics lien is hereby acknowledged on the above vehicle, pursuant to Florida Statutes Section 713.585, to secure the amount of the remaining balance due. No vehicle will be released until payment is made in full.
PRICING & ESTIMATES
I understand that Lions Auto Repair INC's final invoice may not match the insurance company's estimate as to parts, labor, or procedures. Parts prices are based upon recent estimating software and may vary. Labor charges are based upon flat rate hours as is customary in the auto repair industry, not actual time spent on repair. Repair techniques are based upon Lions Auto Repair's professional judgment.
STORAGE CHARGES
Storage charges will accrue 3 days after notification of completion. Inside storage: $150/day. Outside storage: $150/day. Failure to make payment within 20 days will result in commencement of the auction process to satisfy the mechanic's lien.

PARTS & CANCELLATION
I authorize the shop to order necessary parts. Should I cancel after parts are ordered, I agree to a 30% restocking fee. Non-returnable parts must be paid in full.
LIABILITY LIMITATIONS
Lions Auto Repair INC is not responsible for loss or damage to vehicles and/or articles left in vehicles in case of fire, theft, or any cause beyond our control. Prior written notice must be given if the return of used or damaged parts is desired.
ELECTRONIC COMMUNICATIONS & SIGNATURES
I authorize Lions Auto Repair INC to contact me about my repair via phone, email, or text message. My electronic signature has the same legal force and effect as my manual signature, in accordance with the ESIGN Act and UETA.
